FIRST VISIT
What to expect on your first visit
Thank you for visiting the website of Living Dental Health, Dr. Andrew W. Engel, DMD LLC. Our website provides important patient information that will help you become familiar with our practice. It also contains answers to frequently asked questions. Please feel free to call us anytime. We appreciate this opportunity to introduce ourselves and look forward to providing you with quality dental care.
We want you to know that in our office you can expect to be treated with respect at all times. We strive to deliver the best diagnosis and treatment that dentistry has to offer. We believe that these goals can be met through a continuous and open dialogue between you and our entire team.
We encourage you to involve yourself in your own treatment and ask questions throughout our relationship. In our opinion, an informed and involved patient is most likely to achieve the highest level of healthcare, and we look forward to working with you to achieve this goal.
YOUR FIRST VISIT
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a separate day.
Please assist us by providing the following information at the time of your consultation:
- X-rays (if applicable).
- A list of your current medications.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

FINANCIAL POLICY
For your convenience, we accept Visa, MasterCard, Discover, and American Express. We deliver the finest care at the most reasonable cost to our patients, therefore, payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 541-550-5311. Many times, a simple telephone call will clear any misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but this must be accomplished prior to the actual procedure.
DENTAL INSURANCE
Your dental plan is a form of compensation provided by your employer. You can expect the carrier (insurance company) to reimburse you for a portion of our fee. That portion is determined by the contract between your employer and the insurance company. The higher the premium paid by your company, the more generous the reimbursement.
Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you, we will help you process your insurance claims in order for you to receive the maximum benefit. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.
At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you — a contract we feel morally obliged to honor.
INSURANCE, FINANCING AND OUR IN-OFFICE DENTAL PLAN
We believe that everyone has a right to the very best health care. And we believe that a healthy mouth is indicative of a healthy body.
DON’T COMPROMISE YOUR DENTAL CARE. CHOOSE A DENTIST YOU CAN TRUST, REGARDLESS OF INSURANCE.
At Living Dental Health, we are pleased to be in-network with a variety of dental insurance plans. If your dental plan is not one we are in-network for, we can still provide the same excellent services out-of-network. If you don’t see your plan below, give us a call to determine if your family’s treatments are covered.
Payment is due at the time of service unless other arrangements have been made in advance. If you have questions, we are happy to provide clarification. Also, we accept most major credit cards and offer financing through CareCredit as well as an in-office dental plan.
NO INSURANCE? NO PROBLEM!
We are also delighted to offer an in-office dental plan. The plans available range from traditional dental services to include dental cleanings and an exam to more comprehensive dental care for those with conditions that require more frequent visits. Call our office to learn more about the dental plans at Living Dental Health.
